First Appointments
1. Attend your intake appointment:
When your packet has been evaluated by the medical team and the insurance specialists, you will receive a phone call to schedule your intake appointment. This is an all day appointment. We cannot call you to make this appointment until we have completed both evaluations. At that appointment you can expect the following:
- Fasting labs will be drawn (eat and drink nothing after midnight before this appointment)
- Psychological Evaluation which includes testing books and an interview with a clinical psychologist.
- Nutrition Evaluation and nutrition class. Please bring the completed 24 hour diet recall and the Weight and Lifestyle Inventory. We will send you these in the mail the week prior to your appointment.
- Basal Metabolic Rate Study will be done in the surgeon’s office.
- History and Physical examination with a Physician’s assistant.
- You will receive education about your surgical options, lifestyle changes, and exercise requirements to improve your health and prepare you for surgery.
- Meetings with financial counselor in both offices.
*You will be expected to pay $50 to Central Baptist Weight Loss Center for your labs and nutrition evaluation. You may be billed for the balance due on lab work if your insurance does not cover the tests that are ordered by the surgeon. We will discuss your financial obligations for surgery at this time. You will be asked to sign financial documents regarding payment.
* You will be expected to pay the clinical psychologist the co-payment due as required by your insurance. Please call the customer service number on the back of your insurance identification card to determine this amount. This is due at the time of service. If you do not have psychological benefits, you will be expected to make arrangements for payment with the clinical psychologist. The clinical psychologist is not affiliated with Central Baptist Weight Loss Center; he/she is an independent contractor.
* You will be expected to pay $250 program fee to Bluegrass Bariatric Surgical Associates. The remaining $250 balance on your program fee will be collected at your next appointment. Central Baptist Hospital does not receive any of the program fees and your insurance will not cover this fee.
* You will be here ALL DAY. Please bring healthy snacks and a healthy lunch. We ask that you do not bring restaurant food to the Weight Loss Center.
